HEALTH REIMBURSEMENT ARRANGEMENTS
are essentially employer-funded versions of the tax-favored medical savings accounts that have long been promoted for individuals. Under HRAs, employers provide each employee with a personal yearly budget -- say, $750 for an individual -- that can be used for health-care-related services.
As with MSAs, the account is typically paired with high-deductible health plan coverage (for example, $1,500 a year for an individual, with perhaps some preventive services paid for at nearly first dollar). That deductible is partially offset by the HRA budget and partly from the employee's own pocketbook.
What the patient does not spend in any given year rolls over from one year to the next, providing more money to meet the deductible. (Assuring employers that HRAs would not be considered employee taxable income was at the heart of the IRS clarification.) Employers can even let employees use rolled-over HRA funds to pay for health services and coverage after employment ends, an important portability feature.
HEALTH SAVINGS ACCOUNTS (Not Available Yet)
An HSA is a tax-exempt account that you set up with a qualified HSA trustee to pay or reimburse certain medical expenses you incur. A qualified HSA trustee can be a bank, an insurance company, or anyone already approved by the IRS.
Benefits:
- You can claim a tax deduction for contributions you make to your HSA even if
you do not itemize your deductions on Form 1040.
- Employee & employer contributions may be excluded from your gross income.
- Contributions remain in your account from year to year until you use them.
- The interest or other earnings on the assets in the account are tax free.
- Distributions may be tax free if you pay qualified medical expenses.
- “Portable” - stays with you if you change employers or leave the work force.
Qualifying:
- Have a high deductible health plan (HDHP)
- Have no other health coverage.
- Are not enrolled in Medicare.
- Are not claimed as a dependent on someone else's 2004 tax return.

BlueCross BlueShield of Tennessee has recently announced that as part of their continued effort to hold down the cost of health care, they will soon begin requiring prior authorization for high-tech imaging services, such as MRIs, CT scans and PET scans.
Beginning August 1, 2006, providers in Blue Networks P and S will be required to obtain prior authorization for outpatient high-tech imaging services for fully-insured members - both group and individual under age 65. High-tech imaging services include MRIs (Magnetic Resonance Imaging), MRAs (Magnetic Resonance Angiography), MRSs (Magnetic Resonance Spectroscopy), CT (Computed Tomography) scans, CTA (Computed Tomography Angiography) scans, PET (Positron Emission Tomography) scans, and Nuclear Cardiac imaging.
While high-tech imaging services such as these can help doctors diagnose serious diseases, experts agree that these tests many times add little or no information to improve a patient’s diagnosis or treatment. These services are also very costly compared to traditional diagnostic tests such as X-rays.
